Mandatory Reporting and Deadlines
Candidates provisionally allotted seats in this round must report in person to their assigned colleges. The physical reporting window is open from December 2 to December 5 . This step confirms acceptance of the allocated seat and progresses the admission process.
Following physical reporting, candidates must complete and submit a retention status form. This form confirms a candidate’s final decision regarding the allotted seat. The final deadline for submitting the retention status form in person is December 6 , by 5:30 PM . Failure to meet this deadline will lead to seat forfeiture.
During physical reporting, college authorities will verify all submitted documents. Guidelines for In-Service Candidates

All candidates must present the following original documents during the physical reporting period for verification. Prepare these documents in advance:
- Online downloaded NEET PG 2025 application form.
- Copy of downloaded NEET PG 2025 admit card.
- Any valid photo identification proof (e.g., Aadhar Card, Driving License, PAN Card, or Passport).
- NEET PG 2025 mark sheet.
- Nationality certificate, a valid Indian passport, or a School Leaving Certificate (HSC or Class 12) explicitly stating “Indian” nationality.
- MBBS degree or a provisional passing certificate.
- Internship completion certificate, issued by the respective university or the head of the institution.
- The internship completion date must not be later than August 11, 2025 .
- Permanent or provisional registration certificate issued by the Medical Council of India (MCI) or State Medical Council.
- Certificate from the head of the institution, confirming the candidate’s medical college or institute for MBBS is recognized by the National Medical Commission (NMC).
- A medical fitness certificate from a registered medical practitioner.
- Copy of the online fee payment receipt for the counselling processing fee of Rs 3,000 .
- Valid caste certificate, if applying under a reserved category.
Non-compliance with the physical reporting schedule, incomplete document submission, or failure to meet eligibility requirements will result in the automatic cancellation of the provisional seat allotment.
